     Jr. High athletes need to complete a Physical Exam Form (completed at the doctor's office), read the school Athletic Policy and pay the athletic fee and ASB fee. These are turned in to the Athletic Secretary. Coaches will also get a copy of the Athletic Emergency Information Form. It is important that all of items are completed before tryouts or the first practice for their sport. Please call the school for questions at 425-885-7034.      Jr. High 9th grade students competing in high school athletics need to complete the forms from the Lake Washington website below. They can also get the forms at the high school or the jr. high athletic offices.

     Lake Washington School District Athletics web pages contains information about participating in Jr. High sports programs. They also have a printable version of the Physical Exam card used at the doctor's office. Please note: The Volunteer Driver and Authorization for Private Auto Forms do not need to be completed. Athletes are transported by bus from school to the event and back to RJH.
