WASHINGTON STATE HEALTHY YOUTH SURVEY

The survey asks questions that are related to students' health issues.  Results are used to develop and plan programs to support our youth and reduce their risks.  The survey will be given on Thursday, October 16th during advisory period.  It is voluntary and anonymous.  If you or your student do not want to participate in the survey please contact LWHS and they can read quietly while it is given.  There is no wrong answer, it is not a test but please fill it out accurately as it can affect your peers and friends and the services they are provided in the future.

    Request in writing.  $5.00 charge per transcript ordered. 
    Provide your maiden name, graduation date and address where transcript is to be sent.
